Open to patients (16 years of age or older) following discharge from specified hospitals for a mental illness

The goal of this service is to connect with patients who were recently discharged from the hospital for a mental illness to ensure they are functioning well following discharge, have made necessary connections with both formal community resources and informal support networks and have been able to follow their discharge plan. Through this service, patients will be better equipped to care for themselves in their own home environment as they follow their personal discharge plans.
This type of service has been identified by Accreditation Canada as a standard for hospitals to implement and is supported by the Champlain Local Health Integration Network (LHIN).
When patients are discharged following an admission to hospital for a mental illness, they will be offered the option of receiving a Wellness Check call from the Mental Health Crisis Line. Written consent from the patient is required before any Wellness Check call can be made. All Wellness Check calls will be made by Mental Health Crisis Line personnel.
For each patient providing consent, the hospital will provide Mental Health Crisis Line personnel with the patient’s discharge plan along with a signed consent form and appropriate coordinates.
During the Wellness Check call, the worker and patient will cover areas such as present wellness, medication compliance, safety check and referral follow-up. Additional referrals can be provided where necessary. A transfer to the appropriate Mental Health Crisis Team will be made should the patient be found to be in crisis and deteriorating. Should the patient be found to be at imminent risk, emergency services will be contacted.
A reminder that the Mental Health Crisis Line is available 24/7 should the patient wish to use the service will be given at the end of the Wellness Check call.
A worker from the Mental Health Crisis Line will contact each patient who was recently discharged from the hospital for a mental illness and who consented to receiving a Wellness Check call within a period of 3 to 7 days following discharge. Wellness Check calls will be made Monday to Friday, between 10:00 a.m. and 8:00 p.m..
A message will be left on the patient’s voice mail if the worker from the Mental Health Crisis Line is unable to reach the patient and if prior permission has been granted. Each message left on the patient’s voice mail will include a reminder of the availability of the Mental Health Crisis Line and the number to call 24/7.
Presently, the Cornwall Community Hospital and the Pembroke Regional Hospital are participating in this new service. Additional hospitals will be added during the next year with the goal of providing this service to patients discharged from all hospitals in the City of Ottawa, the County of Renfrew, the United Counties of Prescott-Russell, and the United Counties of Stormont-Dundas-Glengarry
